OFFICE HOURS & APPOINTMENTS
Our office hours are
- Monday 8:00 am – 5:00 pm
- Tuesday 8:00 am – 5:00 pm
- Wednesday 8:00 am – 5:00 pm
- Thursday 8:00 am – 5:00 pm
- Friday 8:00 am – 12 noon
The office generally closes at lunch time around 12 noon to 1:00 pm.

To assist in providing you and others more time with the doctor, we would appreciate receiving this information as soon as possible before your appointment date. We are network providers for the following:

- Cigna HealthSpring (not Cigna commercial)
- Humana
- Medicare
- Medicaid (age 20 and under)
- MultiPlan (PHCS, PHCS Savility)
- Railroad Medicare
- Tricare
- Tricare Humana Military
- United Healthcare (not from the Marketplace)
- United Mine Workers (UMWA)
- Viva Health
We are out of network with other insurances such as United Healthcare from the Marketplace, Aetna, and Cigna but can provide care and can file a claim for you.

We are located in the Watermelon Professional Park on Watermelon Road in Northport, Alabama.
WHAT TO BRING
Be prepared to bring the following to your first appointment:
- Your insurance card
- Driver’s license or photo ID
- List of your medications
- Office forms (provided above) unless submitted in advance
